JULY 15TH: PROOF OF VACCINATION BECOMES REQUIRED IN COLORADO

Required Client Forms

In accordance with The Department of Agriculture, Inspection & Consumer Services Division and PACFA requirements and suggestions, all clients, both new and returning, must review and return the following forms, and renew them each year as needed.


The following forms will be required for all pets receiving services by appointment in our facility:

  • Client Information Form
  • Shop Policies Form
  • Document(s) proving all most recent up-to-date vaccinations (including vet/rescue name, vaccination date, and expiration)


We are able to receive paperwork in the following ways:

  • Download, print, and fill out by hand ahead of your scheduled appointment;
  • Download, e-fill the forms using a PDF editor, and email/submit the completed copies;
  • Ask your vet or rescue to email us your pets' vaccination records, if you don't have physical copies available;
  • Or, ask for a pre-assembled packet at your appointment check-in to take home and bring back with you!
