Client Information Forms

In an effort to both get to know your dog better and be as transparent as possible, we please ask all new and returning clients to review and return the following forms in lieu of your pet's next appointment!

Starting on July 1st, 2023, the following forms will be required for all pets receiving services by appointment in our facility:

Client Information Form
Shop Policies Form
Emergency Medical Authorization Form
Document(s) proving most recent rabies vaccination (including vet/rescue name, vaccination date, and expiration)

We are able to receive paperwork in the following ways:

Download, print, and fill out by hand ahead of your scheduled appointment;
Download, e-fill the forms using a PDF editor, and email/submit the completed copies;
Ask your vet or rescue to email us your pets' vaccination records, if you don't have physical copies available;
Or, ask for a pre-assembled packet at your appointment check-in to take home and bring back with you!
