Required Client Forms

As of March 1st, 2026, all clients will be required to acknowledge and submit the following documents:

For dogs, valid and up-to-date vaccination documentation, specifically for rabies, distemper and parvovirus. For cats, valid and up-to-date vaccination documentation, specifically for rabies, feline viral rhinotracheitis (FVR), feline calicivirus, and feline panleukopenia (FPV). This documentation must feature the veterinary clinic, hospital, rescue, or shelter that administered the vaccine to the pet; and the dates of both administration and expiration for all required vaccines.
A signed Client Information & Policies packet (available for download and print below)

You may also be asked to acknowledge and submit additional forms provided for you, such as:

Preferred/Prescribed Items Release
Dematting waiver
Double-Coated Dog waiver
Cat Grooming waiver
